As part of a care management team who will manage complex members, the Care Coordinator will be the primary care manager for a panel of older adult members with a variety of medical and/or behavioral health needs.. If you live within the state of Indiana and can travel to Scott, Harrison, Floyd, and Clark County IN , you will have the flexibility to telecommute. Provide referral and linkage as appropriate and accepted by member (may include internal consult opportunities such as Housing Navigator, Pharmacy Team, Peer Specialist, etc.. Bachelor’s Degree in Social Work, Psychology, Special Education, Counseling, or relevant degree with at least 1+ years of case management experience with older adults, individuals with disabilities or those with serious mental illness. Licensed Practical Nurse (LPN) with a minimum of 3+ years of clinical experience with older adults
Ensures Delivery Associate compliance with local, state and federal regulatory agencies (i.e. DOT, OSHA, etc.). Submits and respond to ideas to improve associate engagement and enablement.. Coordinates efforts with the Safety department to aid in training, conducting associate observations, up-dating preferred work methods, conducting accident investigations to determine root causes and routine safety inspections.. 2 or 4-year college degree in Business Management, Supply Chain Management or similar major preferred.. Exhibit knowledge of spreadsheet software and Word Processing software, and ability to learn Sysco technology software and programs.
