YOUR SCHEDULEOur schedule is designed to promote a healthy work-life balance, ensuring you have ample time for yourself. You'll work the day shift, Monday to Friday, with no nights. THE DIFFERENCE THAT YOU WILL MAKE:In this role, your expertise will have a tangible impact on patient outcomes every day. You will be responsible for providing high-quality, effective management for patients with debilitating chronic diseases. Working closely with the attending physician, you manage acute and chronic conditions, ensuring seamless and comprehensive care. You are an integral part of the interdisciplinary team, communicating and collaborating regularly with the Director of Nursing, social worker, and other key staff to coordinate and improve patient care. Your contributions are vital, and you'll know that you are making a meaningful difference.
REQUIREMENTS: A board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP) 3-5 years of experience as an RN or post-acute role specializing in pulmonary 1+ years of post-acute, long-term, and/or skilled nursing facility experience in pulmonaryBE OUR NEW POST-ACUTE CARE AND LONG TERM CARE APP!
