About the role

CenterWell Home Health is seeking a Home Health Physical Therapist to join our caring team. You will provide in-home physical therapy to help patients regain strength, mobility, and independence, guided by physician plans and patient goals in a home setting. You will document progress, educate PTAs and aides, and coordinate with physicians, the Clinical Manager, families, and care teams to ensure effective care.
