Location:
Ramona, CA - Hybrid. This is a field-based home health position serving patients throughout Ramona, Poway, Rancho Bernardo, Rancho Peñasquitos, and surrounding communities.
Note:
Candidates must have an active California Physical Therapist (PT) license. Home health experience, 1+ year of Physical Therapist experience, a Master's or Doctorate degree in Physical Therapy, BLS/CPR certification, a valid driver's license and reliable transportation, HCHB/Homecare Homebase experience, and OASIS experience are preferred.

Job Description:
Evaluate patients in their homes and assess mobility, strength, balance, coordination, pain, and functional limitations. Develop individualized physical therapy treatment plans based on each patient's needs and goals. Provide therapeutic exercises and techniques designed to improve mobility, strength, function, and independence. Educate patients and caregivers on exercises, safety precautions, and self-care strategies. Monitor patient progress and adjust treatment plans as appropriate. Collaborate with nurses, physicians, therapists, caregivers, and other members of the interdisciplinary team. Complete accurate and timely clinical documentation. Travel throughout the assigned North County San Diego territory to provide high-quality in-home care.
Qualifications:
Active California Physical Therapist (PT) license required. Home health Physical Therapist experience preferred.1+ year of Physical Therapist experience preferred. Master's or Doctorate degree in Physical Therapy preferred. Current BLS or hands-on CPR certification preferred. Valid driver's license and reliable insured vehicle preferred. HCHB/Homecare Homebase experience preferred. OASIS experience preferred.
