Work Environment: As a Physical Therapist at Self-Boost Love Physical Therapy, you will work onsite in various home care settings throughout Fairview, NJ. This role allows for flexibility, including the option to choose your own hours within a Monday to Friday framework, promoting a balance between work and personal life. Key Responsibilities: Conduct detailed assessments of patients' physical capabilities and restrictions to create an individualized treatment plan tailored to their specific needs. Implement a variety of treatment methodologies including therapeutic exercises, manual therapy techniques, and the use of specialized equipment, aimed at alleviating pain and enhancing mobility. Educate patients and their families on rehabilitation techniques and the importance of adherence to treatment protocols, fostering an environment of self-care and proactive health management. Maintain precise documentation of patient evaluations, treatment sessions, progress reports, and adjustments to treatment strategies as needed. Continually adapt therapeutic goals based on patient progress, regularly communicating with team members and stakeholders to ensure cohesive patient care. Recommend and assist in fitting patients with necessary assistive devices for optimized mobility and safety. Participate in regular professional development workshops and stay informed on the latest physical therapy practices and technologies. Patient Population / Client FocusOur patients range from children to older adults, each with diverse needs stemming from injuries, diseases, or surgeries. You will engage with patients in their own homes, which requires not only clinical expertise but also empathy and the ability to adapt treatment to their specific environments and circumstances.

Qualifications: A Doctorate degree in Physical Therapy (DPT) is required, ensuring you have the foundational knowledge necessary to practice safely and effectively. A valid physical therapy license to practice in the state of New Jersey. Exceptional verbal and written communication skills to effectively connect with patients, families, and the healthcare team. Proficient computer skills, particularly in electronic health record (EHR) systems for documentation and communication. Flexibility and adaptability to work varied schedules that meet patient needs.

Preferred Experience: While 2 years of experience is preferred, we are open to considering recent graduates who demonstrate a strong aptitude for home care physical therapy. Experience with a diverse patient population and familiarity with home healthcare environments are advantageous.
