Job Description:
Insight Global is seeking a Community Health Worker to join one of their largest healthcare payer clients. The Community Health Worker serves as a key member of the Regional Care Coordination Team, partnering with members to identify and address healthcare and social service needs while promoting improved health outcomes and overall well-being. This role builds trusting, professional relationships with members through engagement in a variety of settings, including hospitals, provider offices, community agencies, members' homes, and through telephonic or electronic communication. The Community Health Worker supports members by coordinating services and resources, assisting with appointment attendance when needed, arranging transportation and childcare, and providing reminders to help achieve care plan goals. Responsibilities include gathering and verifying member information, contributing to assessments, collaborating with providers, family members, and community stakeholders, and supporting the development and implementation of individualized care plans. The role also assists in delivering health education and wellness resources, maintains accurate documentation in accordance with Care Management guidelines, and advocates for a member-centered approach that empowers individuals to make informed healthcare decisions. Regular travel is required to conduct member, provider, and community visits, while continually identifying opportunities to improve processes and enhance the overall member experience. Additional duties may be assigned as needed to support program objectives and organizational goals.
