UCSF Health, AG is seeking an experienced perinatal RN navigator to lead patient coordination for the EMBRACE Perinatal Integrated Care Program. You will support pregnant and postpartum families, coordinate care transitions, and advocate within UCSF systems to ensure equity-centered care. The role emphasizes clinical psychoeducation, group prenatal care facilitation, and mentorship of junior navigators, with a focus on Black birthing families and culturally rooted practices.
