About the role

Job purpose: A Medical Respite Case Manager guides homeless individuals recovering from illness or injury to stabilize health and housing by assessing needs, creating care/service plans, coordinating medical/behavioral/housing support, connecting to resources (PCPs, housing, benefits), advocating, providing crisis intervention, and ensuring smooth transitions to stable housing, often involving daily check-ins, documentation, and teamwork with clinical/housing staff.
Essential Functions: Assessment & Planning: Conduct needs assessments (medical, behavioral, housing) and develop individualized care/service plans with clients. Care Coordination: Connect clients with services (employment, SSI/SSDI, VA, Medicaid, Food Stamps, Medicare, mental health, substance abuse, etc.), coordinate appointments (sometimes providing transport), and collaborate with clinical teams. Coordinate activities and classes. Case Management: Offer advocacy, crisis intervention, and supportive counseling. Housing Focus: Work with Coordinated Entry and housing staff to develop housing plans and facilitate transitions to stable housing. Client Engagement: Welcome new clients, orient them to the program, provide daily check-ins, and help them adjust. Documentation: Complete intake forms, maintain detailed case notes, and manage HMIS/database entry. Teamwork: Collaborate with other staff, social workers, and community partners. This description reflects management’s assignment of essential functions; it does not proscribe or restrict the tasks that may be assigned.
Qualifications: Valid North Carolina Driver’s License Ability to be added to The Servant Center’s motor vehicle insurance policy. CPR/First Aid Certification Minimum of a year of case management. Minimum of two (2) years working with the homeless or other vulnerable populations Experience working with those with substance abuse and/or mental health diagnoses.
Working Conditions: Most work is performed in a residential setting in an office. Would have to transport clients to appointments occasionally.
Physical Requirements: Requires frequent sitting for long periods and operation of standard office machines and a computer. Lifting up to 25 pounds may be required occasionally. Salary Range$38,000-42,000
