About the role

Patient Transfer Coordinator (CMA, EMT, LPN) The Patient Transfer Center Coordinator is to receive and coordinate calls regarding acute patient transfers, consults and/or admission from medical facilities. Documents the transfers and consults from other medical facilities into transfer center program. The Patient Transfer Coordinator utilizes patient logistics software to coordinate patient flow and bed placement. The coordinator speaks directly to the referring and receiving provider/staff to coordinate patient care. Strong communication skills and knowledge of medical conditions and terminology. Coordinates the hospital rotation for Penn Highlands DuBois.
