About the role
NewYork-Presbyterian is seeking a Clinical Coordinator for Hospice in Hudson Valley. The role requires an experienced RN, NY license, BCLS, and 2+ years in hospice or home care. You will coordinate inpatient hospice care and collaborate with hospital teams to ensure smooth discharges to the Hospice program.
