About the role
Belle, LLC is seeking a compassionate and clinically skilled Nurse Practitioner to provide remote telehealth support for Medicare beneficiaries enrolled in our Chronic Care Management program. You will assess members remotely, offer evidence-based treatment, and collaborate with nurses and care coordinators to keep members healthy and out of the emergency department when appropriate.
