These visits are conversational, consultative, and education-focused—no prescribing, no treatment plans. What You'll Do: Conduct comprehensive in-home wellness assessments Educate members on preventive care benefits Identify potential gaps in care to help reduce avoidable ER visits and hospitalizations Complete documentation using provided EMR tools Visits are face-to-face and take place where members live—allowing for more personalized, impactful care.
Requirements: Active State NP license and Board Certification Valid NPIMinimum 3 months of wellness exam experience preferred Comfort with EMR/documentation systems Ability to travel within a 30–50 - mile radius Bilingual/Spanish Speaking a plus Give on average 2 days per week We Provide Paid training & credentialing support Liability/malpractice insurance Pre-scheduled patient appointments Web-based scheduling & routing toolsi Pad tablet Mileage reimbursement Job Details Job Types: Full-Time, Part-Time, Contract (1099)
