Position Summary: As a Licensed Practical Nurse (LPN) at Brookhaven Center, you will provide hands‑on nursing care to residents while supporting the clinical and leadership needs of the unit. You will collaborate with nursing staff and interdisciplinary teams to ensure residents receive compassionate, high-quality care.

Key Responsibilities:

Provide direct nursing care according to individualized care plans
Administer medications and treatments, ensuring accurate documentation
Perform clinical assessments and monitor changes in residents’ conditions
Assist physicians during rounds and exams
Communicate effectively with residents’ families and care team members
Ensure compliance with safety, health, and quality regulations
Foster a respectful, positive, and collaborative team environment

Qualifications:

Current LPN license valid in NJGraduate of an accredited LPN or vocational nursing program
Ability to work independently and collaboratively as part of a care team
Upon hire provide a valid TB screening
Strong communication, organizational skills, and attention to detail
