Position: Admissions Coordinator
Location: Manassas, VA

We are seeking an experienced Admissions Coordinator to oversee and coordinate the admissions process for prospective nursing home and assisted living residents. This role works closely with residents, families, hospitals, social workers, referral sources, local agencies, and internal departments to ensure a smooth and compliant admissions process. The ideal candidate will have experience in long-term care, hospital case management, Level I screenings, and Medicaid eligibility, along with strong communication and organizational skills.
Key Responsibilities:
- Coordinate the admissions process for prospective nursing home and assisted living residents.
- Assist residents and families with completing admission paperwork and required documentation.
- Enter and maintain accurate admissions, discharges, insurance, and contact information in PointClickCare.
- Maintain accurate census records, bed availability, and resident information.
- Prepare prospective admission summaries for weekly Admissions Committee review, including medical, psychosocial, and financial information.
- Maintain admissions records in compliance with Medicare, Medicaid, licensure, and Auxiliary Grant requirements.
- Coordinate with hospitals, social workers, referral sources, families, and internal departments regarding admissions and returns from the hospital.
- Coordinate transportation needs for residents attending medical appointments.
- Assist with updating admissions policies and procedures to maintain regulatory compliance.
- Prepare admissions, census, marketing, and regulatory reports as needed.
- Arrange facility tours and provide prospective residents and families with admission information and materials.
- Required Qualifications1–2 years of combined experience in:
- Long-term care as an LPN, and
- Hospital case management performing Level I screenings.
- Experience working with the local Department of Social Services Long-Term Care Unit.
- Knowledge of Medicaid and Auxiliary Grant eligibility requirements.
- Knowledge of medical terminology Preferred.
- Detail-oriented with the ability to manage multiple responsibilities.
- Ability to work effectively with geriatric and disabled populations and handle challenging or crisis situations.
- High school diploma or GED required.
