Description: Coverage Territory: Aurora, Wheaton, Naperville and surrounding areas
Schedule: 8 AM-8 PM, alternating 4-day and 3-day weeks including weekends in rotationinclude rotating holidays

What You'll Do: As an RN Case Manager, you'll be the clinical anchor for your patients and their families - coordinating care, guiding the interdisciplinary team, and showing up when it matters most. Your day-to-day includes:
  • Conducting comprehensive, ongoing assessments to understand each patient's evolving needs
  • Building and adjusting personalized care plans in collaboration with physicians and care teams
  • Administering medications and treatments per physician orders
  • Keeping clinical documentation accurate, timely, and meaningful
  • Serving as the communication bridge between families, physicians, and community health partners
  • Educating patients and caregivers so they feel confident and supported
  • Participating in on-call rotation per team policy

Requirements: What You BringActive RN license in the state of employment Graduate of an accredited school of Nursing. One (1) + year Hospice, home health or related organization experience preferred. Compassion for patients and families navigating end-of-life care Strong clinical judgment and the ability to work independently in the field Excellent communication skills across care teams and families
