About the role

Rite of Passage is hiring a Transitional Care Coordinator for the Southern California Treatment Program in Torrance, CA. The role focuses on administering and coordinating case management services for youth, acting as a liaison among residents, families, and agency staff, and guiding youths back into the community. Shifts are Monday–Friday, 9:00 am–6:00 pm. Benefits include medical insurance after 60 days, 403b match after 1 year, and PTO accrual.
