About the role
To support a growing home care team, the part-time California Licensed Home Care Coordinator will conduct in-home care assessments, oversee personal care arrangements, and collaborate with interdisciplinary teams, while enjoying a flexible hybrid work schedule that includes both remote and onsite responsibilities.
Key responsibilities:
Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders and referrals
Communicate effectively with participants and collaborate with nursing, therapy, and social services regarding care updates
Assist with staffing and scheduling activities and participate in end-of-life care coordination
Required qualifications:
Healthcare/medical licensure or equivalency with at least three years of professional experience
Bachelor's degree preferred
Minimum of three years of case management or nursing experience in a clinical or home setting with a frail or elderly population
Nursing knowledge and training necessary to treat frail, elderly participants and care for complicated clinical conditions preferred
