Role Description: The Medicaid Case Manager is a full-time, on-site role based in Cranberry Township, PA, responsible for supporting clients and their families through Medicaid eligibility and long-term care planning. This role includes gathering and organizing financial and personal documentation, preparing Medicaid applications, and coordinating with attorneys, clients, and external agencies. The Medicaid Case Manager will track case progress, maintain accurate records, and follow up on outstanding items to ensure timely submission and approvals. Daily responsibilities also involve meeting with clients, explaining processes and requirements in clear, compassionate language, and responding to questions by phone, email, and in person. The role further requires collaborating with the legal team on estate and asset protection strategies, complying with firm procedures, and contributing to a positive, professional office environment.
Qualifications: Experience with Medicaid, public benefits, or long-term care planning, including understanding of eligibility rules and documentation requirements. Strong case management and organizational skills, with the ability to handle multiple files, track deadlines, and maintain detailed records. Excellent communication and client service abilities, including clear written and verbal communication and a compassionate, professional demeanor. Proficiency with office software (e.g., Microsoft Office or similar tools), data entry, and document management systems. Attention to detail and problem-solving skills to review complex financial information, identify issues, and recommend practical solutions. Ability to work collaboratively with attorneys and staff while managing independent responsibilities and prioritizing tasks effectively. Prior experience in a law firm, healthcare, elder law, or social services setting is beneficial. Associate’s or Bachelor’s degree in social work, paralegal studies, legal studies, healthcare administration, or a related field preferred, or equivalent relevant experience.
