About the role

Kern Medical in Bakersfield is seeking a Care Coordinator for the PCMH program in Reach and Grow LLC Clinics. This full-time, on-site role coordinates patient care across the health care continuum and connects community patients with their primary care providers. The ideal candidate has a Medical Assistant diploma/certification and BLS through AHA. Bilingual is preferred. Compensation ranges from $25.2641 to $33.9528 hourly with potential incentives and 401(k) contributions.
