About the role
- Assess and support members' psychosocial needs
- Develop relationships with members and families and serve as a resource throughout their stay
- Participate in interdisciplinary care planning
- Advocate for member preferences, rights, and quality of life
- Assist members and families with adjustment to long-term care
- Provide support related to changes in condition, grief, loss, and other life transitions
- Coordinate services and community resources as appropriate
- Assist with discharge planning and transitions of care
- Address member and family concerns and help facilitate communication
- Maintain timely and accurate documentation
- Support compliance with applicable long-term care regulations and resident rights requirements
- Active Wisconsin Social Worker license
- Strong communication and interpersonal skillsAbility to navigate sensitive situations professionally and compassionately
- Strong organization and documentation skillsAbility to collaborate effectively as part of an interdisciplinary team
- Long-term care or healthcare experience preferred
