POSITION SUMMARY:
The Nurse Practitioner works in collaboration with a physician and other palliative care team members to deliver palliative care and related health care services to chronic and serious ill patients and families
Essential Duties:
Collaborates with the IDG in assuring appropriate evaluation and palliative treatment of terminal disease
Consults with and assists in the IDGs development of patientfamily plan of care to assure quality care is provided
Consults with and educates the patientfamily regarding disease process self care techniques end of life care
Provides training to other staff as needed
Performs and documents visits to patientfamily based on plan of care and patientfamily need
Interacts with attending and consulting physicians as necessary regarding palliative care in collaboration with hospice physician
Consults with and provides education to other IDG members on a prn basis for clinical issues relating to palliation
Actively participates in the IDG Communicates effectively timely and appropriately at all times
Informs physicians and other personnel of changes in the patients needs and outcomes of intervention while evaluating patientfamily response to care
Qualifications:
Graduate of an accredited School of Nursing and accredited Nurse Practitioner Program
Current state RN and APN license and CPR certification
Minimum 2 years experience as an APN clinical care setting preferred; hospice and home health preferred
Certification in a specialist area preferred; eg Hospice and Palliative Nursing CHPN or Geriatrics
Excellent observation verbal and written communication skill problem solving skills nursing skills per competency checklist
Reliable transportation valid drivers license and current auto liability insurance coverage
