About the role
RN Transitional Care Coordinator St. Leonard | Centerville, OhioSt. Leonard is seeking a compassionate and experienced Full-Time Transitional Care Coordinator to help residents and families navigate their healthcare journey. This position coordinates care across Cottage Recovery, Private Duty HomeCare, and other levels of care while helping residents receive the services and support they need.
Responsibilities:
- Coordinate Cottage Recovery, Private Duty HomeCare, and Wellness Clinic services
- Coordinate admissions, care transitions, and resident services
- Collaborate with physicians, healthcare providers, residents, and families
- Educate residents and families on care options, medications, insurance, and available resources
- Assist with care coordination, documentation, and regulatory compliance
- Connect residents with community and social service resources

Qualifications:
- Bachelors degree in healthcare, social work, or a related field
- Board Certified Patient Advocate
- Experience in senior living, long-term care, or home healthRN and gerontology experience strongly preferred
- Strong communication, organization, and problem-solving skills
