Responsibilities:
Provide occupational therapy services to residents in a skilled nursing and rehabilitation setting
Develop and implement individualized treatment plans to address each resident's unique needs and goals
Conduct assessments, treatment sessions, and evaluations as necessary
Collaborate with other healthcare professionals to ensure comprehensive care and provide education to residents, families, and caregivers
Maintain accurate and detailed records of each resident's progress and treatment
Participate in interdisciplinary team meetings to discuss resident care and progress
