Care Manager (Remote)
Location: Remote (EST hours) Must reside in Connecticut, New Jersey or New York
Employment Type: Indefinite Contract Start: 10/26 - no days off within first two weeks
Schedule:
Monday–Friday, 9:00 AM – 5:30 PM EST 40 hours/week(Includes two paid 15-minute breaks and one unpaid 30-minute lunch)Equipment: Must have your own laptop (cannot be Chromebook or Macbook)Pay Rate 1 year Care Management experience: $21/hr 2 years experience: $22/hr 3 years experience: $234+ years experience: $24/hr The Care Manager supports care coordination efforts for New York Medicaid members by managing referrals, follow-ups, and member engagement. This role focuses on improving health outcomes through proactive outreach, documentation, and collaboration with providers and community partners. Step 1: Cold Screening Participants complete a brief cold screening to identify potential health-related social needs (HRSN's) and determine initial eligibility. Topics may include housing and utilities, food access, transportation, employment, education, and interpersonal safety. Screenings are conducted only with consent, and all information is used solely for eligibility and care planning purposes. Step 2: Eligibility Assessment If unmet needs are identified and the participant chooses to proceed, a care manager conducts a more detailed eligibility assessment. Step 3: Service Connection For eligible participants, the care manager coordinates referrals to appropriate service providers based on identified needs and participant preferences. Step 4: Service Delivery Participants begin receiving approved services. These may be one-time interventions, short-term supports, or ongoing services lasting up to approximately six months, depending on need and program guidelines. Step 5: Follow-Up and Monitoring The Social Care Navigator conducts follow-up to confirm that services were received, assess effectiveness, and determine whether additional support or adjustments are needed.
Required Skills & Qualifications:
High School Diploma or GEDCare management experience Experience with Medicaid and Medicaid patient populations Reliable high-speed internet with Ethernet connection (required)Comfortable with inbound and outbound calling with strong communication and customer service skills Tech-savvy and able to navigate multiple systems Ability to work independently in a quiet, distraction-free workspace
Preferred Qualifications:
Outreach and enrollment experience Bilingual Spanish Experience in remote care navigation or benefit enrollment. Familiarity with healthcare and social service systems in New York State.
