About the role
- Document member interactions, assessments, and outcomes in designated systems.
- Follow established clinical workflows, scripts, decision trees, and escalation pathways.
- Maintain documentation that meets clinical, regulatory, and audit standards.
- Meet established productivity, quality, and documentation expectations.
- Participate in quality reviews, coaching, audits, and other performance activities.
- Work Efficiently in a High-Volume Environment
- Navigate multiple systems simultaneously, including the MTM platform, call systems, email, and documentation tools.
- Manage outbound outreach, documentation, and follow-up activities efficiently.
- Stay current on commonly used medications and program updates.
- Maintain a strong focus on member experience, accuracy, and medication safety.
- Required Qualifications & SkillsActive LPN/LVN license in good standing.
- Experience in a healthcare and/or telephonic outreach environment.
- Comfortable making high-volume outbound calls and working in a metrics-driven environment.
- Ability to work effectively in a hybrid environment, both independently from home and in an office/call center setting.
- Ability to navigate electronic documentation systems and Microsoft Office tools.
- Strong attention to detail and ability to accurately document member interactions.
- Experience with Medication Therapy Management (MTM).Experience with Transition of Care.
- Experience working with Medicare or Medicaid populations.
- Previous experience in a high-volume outbound healthcare call center environment.
- Schedule & Work Environment
- Monday–Friday, 8:30 AM–5:00 PM MST (Arizona local time)
- Monday: Work from home
- Tuesday–Thursday: In-office in Phoenix, AZ 85040 Friday: Work from home
- Hybrid position with a strong emphasis on outbound member outreach and call productivity.$28-29/hour
