Location:
Chelsea NYSetting: Outpatient (FQHC)Specialty: Primary Care + HIVNotes Size: 4-5 exam rooms (smallest site) Patient Flow Daily: 15 (during their first year - they give them a ramp up for the first year on daily patient flow) Usually around 20 Patients per day after first year Clinics are open from 8 to 8:30pm
Schedule:
Three morning/daytime shifts: 8:30 AM – 4:30 PMTwo evening shifts: 12:00 PM – 8:00 PMOn - Call: 2 weeks or 1 week attached to a holiday for a year. (extra pay for this)EMR: EPIC
Requirements:
At least 2 years of experience of primary care exp. Preferred HIV, Trans, LGBTQ etc. DEACan apply Medicaid and Medicare

About the Role:
Clinical Care Conduct patient screenings, examinations, diagnoses, and treatments across the lifespan. Provide care that is respectful, compassionate, unbiased, and consistent with evidence-based guidelines. Deliver care in scheduled 20-40 minute appointments and ensure timely follow-up on patient needs, test results, and referrals. Document all care promptly in the electronic health record (EPIC).Provide primary care services Collaboration & Coordination Make appropriate internal and external referrals. Work effectively within a team-based care model, collaborating with other healthcare professionals to ensure coordinated patient care. Quality & Professional Development Participate in quality improvement initiatives and case conferences. Engage in ongoing professional development opportunities, including continuing medical education.
