PruittHealth seeks a licensed Physical Therapist to provide one-on-one home health services to adult and geriatric patients. In this field-based role, you will perform comprehensive assessments, develop and implement individualized treatment plans, deliver evidence-based rehab interventions, and educate patients and caregivers to maximize safety and independence. Collaborate closely with nurses and physicians, document care in EMR, and manage your schedule across a local territory. Provide in-home physical therapy evaluations and develop individualized care plans Deliver evidence-based therapeutic interventions to improve mobility, strength, and function Educate patients and caregivers on exercises, safety, and use of adaptive equipment Document assessments, treatments, and outcomes accurately in EMR systems Collaborate with nurses, physicians, and other clinicians on coordinated care plans Monitor patient progress and adjust treatment plans based on clinical outcomes Manage a field-based schedule and travel between patient homes in assigned area Ensure compliance with Medicare, payer, and home health regulatory standards Physical therapy evaluation and treatment planning Home health care documentation (EMR) Therapeutic exercise and gait training Pain management techniques Patient and family education Care coordination with interdisciplinary teams Fall risk assessment and prevention Wound care and mobility support Time management and independent scheduling Knowledge of Medicare and home health regulations Medical Insurance Dental Insurance Vision Insurance Life Insurance 401k Active state Physical Therapist (PT) license Doctor of Physical Therapy (DPT) preferred CPR/BLS certification Home health experience preferred Valid driver’s license and reliable transportation
