Requirements: Board Certified OR Board Eligible Within 5 Years of Residency - Required Accepted Specialties: Neurosurgery, Neurology, or Neuroradiology Fellowship - Required Active Connecticut License in Hand at Time of Submission - Required, No ExceptionsDEA - Required Connecticut CSR/CDS - RequiredBLS - RequiredACLS - Required Credentialing Temporary Privileges Available Within the Needed Timeline Connecticut License Must Be in Hand at Submission Credentialing Costs and Application Fees Are Not Reimbursed
