About the role
Pneuma Care Health and Wellness is seeking a Community Care Coordinator to collaborate with the CalAIM ECM/CS programs. The role involves engaging members where they live, coordinating care plans, and ensuring seamless service delivery with MACC oversight. The incumbent will facilitate assessments, update plans, arrange transportation, and work with providers to meet health and psychosocial needs. Travel to client sites is required.
