Role Description:
The Lead Case Manager is a full-time, on-site role located in Warrensville Heights, OH. This role oversees case management services for program participants, ensuring individualized support plans are created, implemented, and monitored. The Lead Case Manager conducts intake assessments, maintains accurate case records, coordinates referrals to internal and external resources, and tracks client progress toward goals such as education, personal development, and community engagement. The position includes supervising or mentoring other case managers or volunteers, collaborating with program staff to align services with organizational mission, and participating in team meetings and community outreach activities. The Lead Case Manager also communicates regularly with families and partners, prepares reports, and helps improve case management processes and program outcomes.
Qualifications:
Demonstrated experience in case management, social work, counseling, or youth services, ideally working with girls or adolescents. Strong skills in assessment, care planning, and coordination of community resources to support client goals. Excellent communication, active listening, and interpersonal skills to build trust with participants, families, and partners. Organizational and documentation skills, including maintaining accurate records, preparing reports, and managing multiple cases. Leadership abilities to guide, mentor, and support staff or volunteers in delivering high-quality services. Commitment to empowering youth, promoting integrity, self-love, discipline, and educational attainment in alignment with M.E. INC’s mission. Ability to work on-site in Warrensville Heights, OH, with reliability and professionalism in a collaborative team environment. Bachelor’s degree in Social Work, Psychology, Counseling, Human Services, or a related field preferred; relevant experience may be considered. Knowledge of local social service and educational resources and basic proficiency with computer systems and case management tools.
