What You'll Do: You'll provide comprehensive psychiatric care while working collaboratively with behavioral health clinicians, primary care providers, and other members of the healthcare team.
Responsibilities include: Conduct comprehensive psychiatric evaluations and assessments Diagnose mental and behavioral health conditions Develop individualized psychiatric treatment plans Provide ongoing psychiatric medication management Educate patients and families regarding medications, potential side effects, and treatment options Provide therapeutic support and counseling within scope of practice Assess patients experiencing behavioral health crises and coordinate appropriate interventions Treat children, adolescents, and adults with a variety of behavioral health needs Support patients with co-occurring mental health and substance-use conditions Collaborate with primary care and behavioral health clinicians Coordinate referrals to specialists and outside behavioral health services when appropriate Monitor patient progress and modify treatment plans as clinically indicated Maintain accurate and timely clinical documentation Participate in multidisciplinary care planning and case discussions Advocate for patients and help connect them with appropriate behavioral health resources
What We're Looking For: Board-certified or board-eligible Psychiatric Mental Health Nurse Practitioner (PMHNP)Active and unrestricted New York State Nurse Practitioner license 2+ years of psychiatric/behavioral health experience preferred Comfortable treating children, adolescents, and adults Experience with psychiatric assessment, diagnosis, and medication management Strong clinical judgment and crisis-assessment skills Comfortable working collaboratively with behavioral health and primary care teams Experience with substance-use or co-occurring conditions is a plus Bilingual English/Spanish preferredNYS Medicaid provider status is a plus Why Consider This Opportunity?
