About the role

Director of Care Transitions and Partnerships Uni Cura Health, Morgantown, WV. Full time, reports to the Founder. Uni Cura Health is a new home care agency opening November 1 in Morgantown. We employ our caregivers, guarantee their hours, start within 48 hours of a referral, and send the referring case manager a written report after the first three days. We are hiring the person who owns our relationships with the people who decide where a patient goes when they leave the hospital.
What you'll do: Open and hold referral relationships across Monongalia, Marion, Harrison and Preston counties: hospital case managers and discharge planners, SNF social workers, home health and hospice, physician practices, elder law attorneys and financial planners Be the agency's answer to the difficult Friday afternoon discharge: a same-day yes or no, a start within 48 hours, and follow-through so the case manager never has to chase us Own the written report back to every referral source after the first three days of care Set the referral-to-start process with our Executive Director and Care Coordinator and fix it when it breaks Carry a census target, tracked quarterly, with the markers written into your offer letter Year two: sell the same desks a per-case transition block for the patient who could go home today if someone were there, and open the care-management conversations with physician practices Who this is forA director or manager of case management, a hospital or SNF social work lead, or a liaison who has outgrown the role Someone who has spent a career on the receiving side of home care referrals and knows which agencies get the call and why Knows the discharge planners in this market by name Five or more years in care transitions, case management, post-acute admissions, or healthcare referral development in North Central West Virginia Licensed clinical background (RN, LSW, LPN) preferred, not required Comfortable building a function from nothing with a founder who will give you decision rights and expect you to use them
