About the role

The Community Hospice, part of Trinity Health, has a full-time social worker opportunity for our hospice care team serving patients in Columbia County and surrounding areas. You will join the interdisciplinary team to support patients and families in homes and nursing facilities, focusing on emotional and psychosocial support. Responsibilities include counseling, therapy, and resource coordination.

Requirements include: NYS SW Licensure, a valid NYS driver’s license, and 2–3 years of experience
