Optimal Care is seeking a Physician Services clinician to deliver advanced primary and specialty care to geriatric and medically complex patients, often in their homes or care settings, with autonomy to emphasize thoughtful, patient-centered care that improves comfort and quality of life. You will perform comprehensive evaluations, diagnose and manage conditions, develop individualized treatment plans, coordinate with patients, families and interdisciplinary teams, and provide medical leadership
