About the role

Hours: 40 hours per week; Monday - Friday, 8:00 am to 4:30 pm (no on-call, evenings, or weekends)

Pay Rate: $28.01 to $29.48 per hour

Education: Bachelor’s degree in Public Health, Social Work, Psychology, Human Services or related field with 2 years of experience working in social services or related setting. Responsibilities:

Provide mobile outreach and engagement
Assist leading care management and case conference discussion
Screen and assess for program eligibility
Promote positive health outcomes for clients utilizing mobile case management, care coordination with other providers, and supportive services with a team of dedicated professionals
Connect clients and their families to a medical clinic and providers, as well as other clinic and community‑based services
Arrange transportation and accompany clients to office visits as needed
Serve as an advocate for the client and family
Use motivational interviewing, trauma‑informed care, and harm‑reduction practices
Create and maintain all required documents in the electronic and paper record systems
Qualifications and SkillsAt least six months of experience working with youth strongly preferred
Experience working with Seriously Mentally Ill (SMI) population is preferred
A valid California driver’s license in good standing, personal vehicle, proof of insurance and eligibility to be a CRF Approved Driver are required to fulfill the essential functions of this position
Ability to pass criminal/background clearance
