About the role
- Provide comprehensive case management services to Veterans, including assessments, service planning, and ongoing support.
- Develop individualized service plans based on Veterans' needs, goals, and strengths.
- Monitor and evaluate Veterans’ progress, adjusting service plans to ensure adequate support.
- Effectively utilize Electronic Health Record (EHR) systems to document case notes, service plans, and all relevant Veteran interactions.
- Coordinate with community resources, service providers, and support networks to meet Veterans' needs.
- Conduct regular visits and check-ins to assess living conditions, wellness, and compliance with program expectations.
- Advocate for Veterans to ensure access to necessary healthcare, employment, and benefits.
- Maintain accurate and up-to-date case files, documentation, and reports, ensuring data integrity and confidentiality.
- Facilitate life skills workshops, employment readiness training, and financial literacy sessions.
- Collaborate with multidisciplinary teams, including entitlement and employment professionals, behavioral health specialists, peer support, and property management, to support Veterans' stability and development.
- Provide crisis intervention and de-escalation support, connecting Veterans with emergency resources when needed.
- Assist Veterans with navigating housing retention strategies, eviction prevention, and conflict resolution.
- Ensure compliance with program policies, procedures, and regulatory requirements while maintaining a strengths-based and trauma-informed approach.This is a Part-Time position (20 hours) with potential for Full-Time.Requirements
- Bachelor’s degree (B.A.) in Social Work, Psychology, Human Services, or a related field.
- Master’s degree in Social Work or a related field preferred.
- Minimum of 3 years of case management experience in a supportive housing, social services, or related environment.
- Strong understanding of housing-first models and trauma-informed care.
- Experience utilizing Electronic Health Record (EHR) systems.
- Excellent communication, interpersonal, and organizational skills.
- Ability to work independently and as part of a multidisciplinary team.
- Strong problem-solving skills and the ability to manage multiple priorities.
- Commitment to promoting diversity, equity, and inclusion.
- Experience working with veterans, individuals experiencing homelessness, and/or vulnerable populations.
- Knowledge of local community resources and support services.
- Proficient in Microsoft Office/ Google Suite (or equivalent).Pre-employment screening, including a background check, is required.*Required to work on site *This is a Part-Time position (20 hours) with potential for Full-Time.Working conditions
