Admin Support (Non-Clinical) Henry Ford Macomb - Oakland Hospitals2611873 Requisition #, Full time Days - 9am-5pm, no weekends The NRS Community Monitored Living Program (CMLP) is a program that provides support to individuals with traumatic brain injuries living in their own home. This position serves as a case manager providing individualized programming including but not limited to assessment and training in advanced activities of daily living, attendance at medical appointments, coordination and follow through of medical directives, and assistance in medication obtainment and follow through.
Responsibilities:
  • Investigates, assesses and plans interventions to help patients cope with social, emotional, economic and environmental problems.
  • Interviews patients, assesses priorities and documents case activity.
  • Consults with team managers, statutory and voluntary agencies and patient's relatives to ensure compliance with patient's treatment plan.
  • Assists clients/families with obtaining community assistance by referral to proper resources.
  • Develops and maintains working relationships with community health, welfare and social agencies and seeks creative means to assist the client with his/her needs.
  • Supports management of concurrent review, discharge planning processes, social services, home care and other post discharge needs.
  • Bachelors degree in social work
  • Experience with TBI patients preferred
  • Ability to travel to patient homes
