About the role
- Conduct diagnostic assessments and initial mental health evaluations
- Provide follow-up psychotherapy to residents in SNFs and ALFsMaintain clinical documentation in compliance with state and federal regulations
- Collaborate with facility staff and Directors of Mental Health
- Participate in treatment planning and interdisciplinary care coordination when needed
- Active LMFT, LMHC, or Psychologist license
- Masters or Doctoral degree in mental health counseling, marriage and family therapy, or psychology
- Experience working in behavioral health or long-term care preferred
- Ability to work independently with remote support
- Strong documentation and communication skills
