About the role
This is a Contract-to-Hire Physical Therapist opportunity with an established Home Health/Clinic organization located in Brooklyn. The ideal candidate is a skilled professional who is passionate about delivering quality care and making a real difference. The Physical Therapist oversees plans and administers Physical therapy treatment programs for medically referred patients to restore function, relieve pain, and prevent disability following disease, injury, or loss of body parts.
Schedule:
8:30 AM - 5:30 PM - M-FLocation: Brooklyn, NYSetting: Field (10%) - On-Site (90%)
Responsibilities:
Assess each patient and develop an appropriate treatment plan with measurable goals and time frames Perform participant evaluations and treatment at the outpatient clinic and/or in the field (participant’s home, nursing homes, etc.)Prepare and submit clinical and progress summaries based on identified and attained goals Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
Qualifications:
Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum Two (2) years of experience preferred. Certification in Physical Therapy and valid NY State license to practice Valid NYS driver’s license and reliable vehicle required
