Hospice and Home Health Community Liaison Santa Clara County, CA.Full-time.
Company: Blize Healthcare. Blize Healthcare is seeking a compassionate and driven Hospice/Home Health Community Liaison to join our team. In this role, you will build and maintain relationships with referral sources, including physicians, hospitals, skilled nursing facilities, and community organizations, to support patient access to hospice care.
Responsibilities: Develop and maintain strong relationships with healthcare providers and community partners. Identify and secure new referral sources through targeted education and outreach efforts. Represent Blize Healthcare at trade shows, community events, and professional gatherings. Educate the community about hospice and Home Health care and services. Monitor and analyze referral and admissions data to identify trends and opportunities. Collaborate with clinical teams to ensure seamless patient transitions to hospice and Home Health care.
Requirements: Strong communication, interpersonal, and presentation skills. Proven ability to build and maintain professional relationships. Sales or marketing experience, preferably in the healthcare industry. Knowledge of hospice and Home Health care continuum, services, and regulatory requirements. Associate's degree or equivalent experience. Ability to lift to 50 pounds as needed. Reliable transportation for travel within the assigned territory.
