About the role

Habitat Health seeks a Home Care Coordinator to ensure personal and clinical homecare needs are delivered for participants in their homes. As part of the IDT, you’ll assess needs, develop care plans, and coordinate services to support safe, person-centered care in home settings. You will conduct comprehensive home assessments, collaborate with families, and manage authorizations and documentation while traveling between sites to meet participant needs.
