Position: LCSW/LPC
Schedule:
Monday – Friday, 30–40 hours/week (typically between 9:00 AM – 5:00 PM, flexible)
Pay Rate: $45.00/hour
Work Model: Field-based (travel to facilities) with remote charting flexibility
Position Overview:
We are seeking licensed clinicians to provide behavioral health services within skilled nursing and assisted living facilities. This role combines direct patient care, care coordination, and administrative responsibilities, with strong autonomy and flexibility in managing daily schedules.
Key Responsibilities:
Clinical & Patient CareConduct Health Behavior Assessments (HBAI) to evaluate psychological and emotional factors impacting physical health
Provide individual psychotherapy and counseling services to residents
Manage an active caseload of approximately 20 patients
Develop individualized treatment plans and clinical recommendations
Care Coordination & CommunicationCollaborate with nursing staff, administrators, and interdisciplinary teams
Provide updates and coordinate care plans across providers and facilities
Administrative & ComplianceComplete accurate EMR documentation for sessions and assessments (can be done remotely)
Track client contact hours and maintain up-to-date documentation for timecard submission
Ensure compliance with HIPAA, state licensing requirements, and professional standards
On-Site OperationsTravel to multiple skilled nursing and assisted living facilities across the Metro Atlanta area
Follow facility protocols, including check-in procedures and badge requirements
Required QualificationsActive Georgia license ( LCSW or LPC)
Ability to pass background check and provide current TB test
Strong communication, organization, and documentation skills
Preferred Qualifications:
NPI number and/or Medicare registration (or willingness to obtain post-hire)
Travel TerritoryMonroe, GA (~45 miles east of Atlanta)
