Full-Time Hospice/Home Health Social WorkerFull-time hospice/home health social worker based in Spokane, WA. Provide emotional, social, and practical support to patients and families, ensuring holistic, patient-centered care throughout their journey.
Key Responsibilities:
  • Conduct psychosocial assessments for hospice and home health patients
  • Provide emotional support, counseling, and crisis intervention for patients and families
  • Assist patients and caregivers with advance care planning and end-of-life support
  • Connect patients with community resources, including Medicaid assistance, nutrition programs, transportation, caregiver support, utility assistance, and housing services
  • Coordinate respite care services and hospice transfers as needed
  • Collaborate with nurses, physicians, chaplains, and interdisciplinary team members
  • Advocate for patient rights, dignity, and quality of life
  • Maintain accurate documentation in compliance with hospice and home health regulations
Qualifications:
  • Bachelor's or Master's degree in Social Work from an accredited program
  • Active Idaho Social Work license
  • Licensed Master Social Worker (LMSW) required
  • Knowledge of community resources, Medicaid, and social service systems
  • Strong communication, organization, and time-management skills
  • Valid driver's license with the ability to travel to patient homes
Preferred Skills: Experience in hospice, home health, medical social work, or healthcare setting
