About the role

The role collaborates with nurses, physicians, and insurers to navigate post-acute care and community resources. Responsibilities include assessing psychosocial needs, developing discharge plans, coordinating care teams, and educating patients and families on options and coverage. A BSWS and related experience are preferred.
