Job Title: Licensed Master Social Worker
Location: New York, NY 10016 Shift Time: 9a-5pDuration: 3 Months with strong possibilities of extension
Required Skills: Experience working with Inpatient Med surg Manager would like someone with Hospital experience At least 1 year of experience
Required Education: Master's Degree in Social Work Valid Licensed Master of Social Work (LMSW) license issued by NYSED required Interview: Web Ex/phone
Duties: This position is in inpatient medicine/surgery. Attend team/unit rounds Utilize Allscripts, Careport, Parachute applications for referrals/orders As needed, ensure all patients are notified of their discharge in accordance with time frames and documentation specified in the Departmental policies Meet with all patients on caseload a minimum of once per week, unless clinically inadvisable (eg pt too disorganized to tolerate) in which case meetings should be adjusted to accommodate pts need. Provide and review discharge plan and aftercare referral form to patient of family as indicated Complete screening, assessments, notes, care plans, education within time frames in the EMRParticipate as member of interdisciplinary team. Develop comprehensive and safe discharge and after care plans with team. Develop and maintain relationships with community agencies Execute discharge plans, including referrals for home care and Skilled Nursing Facilities
Required Education: Master's Degree in Social Work. Required Certifications & Licensure: Valid Licensed Master of Social Work (LMSW) license issued by NYSED, Primary Source Verification.
