About the role

The RN Coordinator will report into the Executive Director of Nursing Operations and provide resident assessment at pre-admission, quarterly, annually, and when a change in health condition occurs in accordance with federal/state regulations.•Assists with the coordination and maintenance of all required schedules for MDS/Resident Assessment Instrument (RAI) for residents upon admission, quarterly, annually, with condition changes, and following current CMS Medicare/Managed Care schedules.•Assists with the coordination of assignment of support documentation for RAI to all departments. Assess changes in resident condition daily and coordinates new RAI processes when applicable.•Coordinates all applicable support documents including codes, batch checks, MDS printing, and locking procedure for RAI within time frames and in accordance with state and federal regulations.•Assists the team with reviews, assessment and documentation for potential new admissions prior to admission and provides information to applicable staff.•Coordinates care plan schedules/standards and ensures compliance through education/review/audits.•Coordinates Medicare utilization coverage with all applicable departments as required. Facilitates weekly review meetings.•Coordinates/verifies resident CMI for quarterly picture dates.•Coordinates the restorative program. Performs resident documentation as required.
Job Requirements: Current Licensure as a PA Registered Nurse. Current cardiopulmonary resuscitation (CPR) certificate.
Please Note:
  • Three (3) to five (5) years directly related experience in a long-term healthcare center.
  • Demonstrated knowledge of the laws and regulations regarding MDS, Medicare, Managed Care, Medicaid case mix, current RUGS, and the resident classification system.
  • Ability to lift and turn residents with a maximum of 50 lbs. effort and perform the functions required to push residents in wheelchairs.
  • Ability to work in a constant state of alertness and safe manner.
  • Fit testing as needed
  • Exposure to Hazardous Conditions:blood and body fluids are occasionalcontaminated needle sticks are occasional
