Growing together.As the Social Worker - Per Diem, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.Primary Responsibilities:Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basisContributes as a health care team member to the development of a comprehensive, integratedPlan of Care for patients on a daily basisInstructs health care team members on community resources available to assist patients on a as needed basisAble to function as Bereavement Coordinator and supervise the provision of bereavement services reflective of patient family if needed. Establishes a Plan of care that addresses bereavement needs with clear delineation of services to be providedYou'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Required Qualifications:Bachelor of Social Work OR Masters of Social WorkCurrent Driver's License, vehicle insurance, and access to a dependable vehicle or public transportationPreferred Qualifications:Current CPR certification or ability to complete within 90 days of hireLicensed Social Worker in the state of practice 1+ years of social work experience in a health care settingBereavement Coordination experienceExperience with establishing a plan of care for bereavement needsPay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
