Job Summary: This role is a mixed position consisting of both inpatient clinical staffing and transitions of care responsibilities. Our transitions of care team focuses on reducing hospital readmissions through medication reconciliation, discharge medication education, identifying and resolving medication access barriers, and supporting safe and effective medication use following discharge, with a particular focus on patients with heart failure and other chronic disease states. Inpatient clinical staffing responsibilities will include a mix of medical/surgical patient care areas. Responsible for the provision of comprehensive pharmaceutical care in a patient-focused manner that supports and promotes medication safety. Assignments may vary based on departmental needs and the specific training and skill set of the individual employee. Orients and trains pharmacists, technicians, residents, interns, and students as assigned.
Required Qualifications:
  • Require active Florida Pharmacy license. If no active Florida Pharmacy license, must be registered as a Pharmacy Intern in the State of Florida and have an active Florida Pharmacy license within three (3) months of hire.
  • Require the ability to obtain a level 2 AHCA clearance.
Preferred Qualifications:
  • Prefer recent institutional practice experience as a pharmacy intern, student, or registered pharmacist or other comparable professional qualifications. Employment Screening Requirements
