The Homecare/Community Liaison Worker supports people with disabilities and older adults by connecting them with multi-service support, health services, and social care services. The role involves daily home and community client outreach, community referrals, provider coordination, and follow-up support, with progression opportunities to Senior Liaison Worker.
Responsibilities:
Conduct client outreach with people with disabilities and older adults. Navigate community resources and make appropriate referrals. Coordinate multi-service support, health services, and social care services. Act as a liaison between clients, service providers, and community organizations. Build effective relationships with clients, providers, and community partners. Provide follow-up support to monitor referrals and ongoing service needs.
Preferred Qualifications:
2+ years of experience in community care coordination. Experience with community resource navigation. Strong communication skills. Relationship-building skills.\nCompany
