Director of Rehab SLP
Job Summary: St Johnsbury Center for Living and Rehabilitation, a premier residential, long-term, rehabilitation, and subacute care provider, is seeking a compassionate and experienced Director of Rehab SLP to join our team. As a vital member of our rehabilitation department, you will play a crucial role in delivering high-quality patient-centered care. Located in Lyndonville, NY, our state-of-the-art facility offers a luxurious environment that encourages comfort and a sense of wellbeing. Responsibilities:
  • Develop, implement, and oversee rehabilitation programs for patients with a range of conditions, including neurological, musculoskeletal, and cardiopulmonary disorders
  • Collaborate with multidisciplinary teams to ensure comprehensive care and optimal patient outcomes
  • Administer and score various speech and language assessments to identify client needs and develop individualized treatment plans
  • Provide individual and group therapy sessions to clients, focusing on cognition, speech, and language skills
  • Conduct evaluations and assessments to identify client strengths, weaknesses, and potential for improvement
  • Develop and implement strategies to improve patient outcomes, including discharge planning and case management
  • Ensure compliance with all regulatory requirements, including CMS and state-specific regulations
Requirements:
  • Master's degree in Speech-Language Pathology (CCC-SLP) and current licensure in the state of New York
  • Minimum 3 years of experience in a rehabilitation setting, with a strong background in speech and language therapy
  • Proven track record of improving patient outcomes and working effectively as part of a multidisciplinary team
  • Excellent communication, organizational, and leadership skills
  • Ability to work in a fast-paced environment and adapt to changing priorities
